Service coordination

Central Adelaide and Hills Partners in Recovery (CAHPIR) assists services to work in a more collaborative, coordinated and integrated way for people with both severe and persistent mental illness and complex needs, and their carers and families.
We work in partnership with the community, primary health care providers and partner organisations to provide high quality, person-centred, equitable and accessible support.
We work with people to identify how services could be better coordinate and strengthen the relationships between their services.

Partners in Recovery is funded by the Federal Government through Neami National.

 

Eligibility

The eligibility criteria varies for our different programs. Please contact this service to discuss whether our programs are suitable for you.

 

Referrals

Referrals can be made by individuals, carers, families, mental health professionals or other health care providers.

Referrals must be discussed with the individual prior to making the referral.

We encourage referrals from people from culturally and linguistically diverse backgrounds, Aboriginal and Torres Strait Islander people, youth and refugees.
